Total Health Assessments
Want to learn more about your health?
Take your first step towards a healthier life by taking the free, personalized online health assessment called the Total Health Assessment (THA). In about 20 minutes, you’ll have a better understanding of your own unique situation – what’s going on with your health and what you can do to improve it.
What is an online health assessment?
The Total Health Assessment is a series of questions designed to help you identify your potential health risks. It will also help you establish attainable, measurable goals to change the behaviors that may be keeping you from having the best possible health. Based on your responses, you’ll receive immediate feedback in a personalized report that includes your current health status, your health risks, and suggestions to help you improve your overall health and well-being.
Is participation mandatory?
No, participation is voluntary. We encourage employees enrolled in a medical plan to complete the Kaiser Permanente Total Health Assessment. You may also elect to have your spouse and dependents over age 18 (who are covered on a medical plan) take it, but it is not required.
What type of questions will I be asked?
Most of the questions will be focused on finding out more about your lifestyle. There are also a few health-specific questions. To receive the most accurate report, be ready to enter recent biometric test results including height, weight, blood pressure, LDL/HDL/total cholesterol, triglycerides and glucose results.
What does Kaiser Permanente do with the health assessment results?
The information you share will remain completely confidential with Kaiser Permanente, and your privacy will not be compromised in any way. Your employer will be notified when you complete the health assessment, but will not be given the details of your individual health history.
Why is completing the Total Health Assessment important?
Your results will be combined with the responses of other employees and included in an aggregate report that indicates potential health challenges employees may be facing as a group, not individually. Future wellness programs and initiatives may focus on these challenge areas in an effort to improve the overall health of employees.
1. Visit kp.org, and sign-in with your user ID and password. If you do not have an ID and password, you will be prompted to create one.*
2. Hold your mouse over the “My health manager” tab; click on “My medical record”; then click on “Test results” and “Past visit information” and write down your test results.

If you have not had these done, skip to the next step.
3. Next, hold your mouse over the "Health and wellness” tab; click on “Programs and classes;” then click on “Total health assessment” to the left of your screen.
4. Click on “Total Health Assessment.” If this is your first time on the site, you must agree to the “Terms and Conditions” before moving forward with completing the assessment.
5. Click “Go” on the “Total Health Assessment” page and complete the assessment.
*Note: If you are a first time user or have forgotten your password, you will need to answer a set of security questions in order to obtain your password. If you are unable to answer the questions, a temporary username and password will be mailed to your home within 3-7 business days.
Want to share this action plan with your KP health care team? Simply click on the option to share your information with them when you complete the online questionnaire, and a summary will be placed in your electronic medical record. When you come in for your next visit, you’ll be able to discuss all the changes you want to make with your personal practitioner.

Hours: 5:30 a.m. – 6:30a.m.
Lori Marten will open the doors at 5: 30 a.m. – Swim time will be for (1 hour) closing at 6:30 a.m.
All employees and guestsover 18 years of age who use the swimming pool must sign a waiver and submit it to Lori Marten,Wellness Coordinator,Human Resources.
Swimmers must be 27J employees and their guests 18 years of age and older.
